According to the policies of the Archdiocese of San Antonio, students are not allowed to carry any medications on their person. The principal will designate a responsible person to supervise the storing and administration of medications at the school. Medications may be administered by non-medical personnel in the absence of the school nurse. The school and employees of the school will be held harmless for adverse drug reactions and side effects of properly administered medication. 

The parent/guardian listed above gives St. Anthony Catholic High School personnel permission to provide their son/daughter over the counter medication.

I further agree that my signature on this document is as valid as if I signed the document in writing. I am also confirming that I am authorized to enter into this Agreement. If I am signing this document on behalf of a minor, I represent and warrant that I am the minor’s parent or legal guardian.
