Failure to complete this form will result in a default YES for each of the five items below.
According to the Family Education Rights and Privacy Act (FERPA) Federal law, certain information about students is considered directory information and will be released to anyone who follows the procedures for requesting the information unless the parent or guardian objects to the release of the directory information. If you do not want St. Anthony Catholic High School to disclose your child’s director information without your prior written consent, you must notify the school in writing by the end of the first 10 days of instruction. Failure to complete this form will result in the release of directory information upon request. (Refer to Section 6: Academic Policies and Procedures)
A parent is allowed to record their objection to the release of all directory information on one or more specific category of directory information. St. Anthony Catholic High School has designated the following information as directory information:
•Student’s name, address, and telephone number
•Photograph, participation in officially recognized activities and sports, and weight/height of athletic teammembers.
•Dates of attendance, grade level, degrees, honors, major fields of study, awards received in school, and mostrecent school attended.
